2012 CHCI Hispanic Heritage Month Events Individual Registration Form
*Groups of individual registrants larger than 15 can benefit from a 10 percent promotional rate through August 31, 2012. To receive the promotional rate, all individual registrants within a group must register together, with one single payment for the entire group at time of registration, and group registration must be completed through CHCI’s Events Department.

CANCELLATION POLICY: Cancellations will be accepted through August 17, 2012, and a $50 cancellation processing fee will apply. From August 17 forward, all sales are final. Refunds will be issued less cancellation fees within 10 days following the event. All refunds requests must be submitted in writing to the following: 1- Full package registration includes: Chair’s HHM Kick-Off Reception (9/10/12); All Summit & Plenary Sessions; Tuesday Continental Breakfast (9/11/12); Opening Luncheon (9/11/12); Arts & Culture Reception (9/11/12); CHCI Alumni Association Next Generation Professional Development Session (9/12/12);Wednesday Continental Breakfast (9/12/12); Closing Luncheon (9/12/12); One CHCI 35th Anniversary Awards Gala Ticket (9/13/12), including access to Gala General Reception & After Party.
